Please keep medical details out of web forms

Our website forms are for scheduling and general questions only. They are not a secure channel for health information. Please don't include symptoms, diagnoses, medications or insurance ID numbers. We'll follow up by phone or text and handle the details there or in the office.

Health information you share with us as a patient is protected under our Notice of Privacy Practices, which is available at the front desk.

How we use it

Who else sees it

Form submissions are delivered to our front desk through service providers that host our website, store the messages and send them by email. They process the information on our behalf and only to deliver it to us. We don't sell or rent your information, and we don't share it for marketing.

Third-party content

Some pages embed content from other services, like Google Maps on our contact page. Those services may collect information under their own privacy policies. Links to our social media profiles and review sites take you to sites with their own policies too.

How long we keep it

We keep form messages only as long as we need them to respond and keep reasonable records, then delete them.
